Alternate Transportation Form
Use if student athlete will not be transported with the team. Complete each line in the order it appears. Student takes the form to the Parent and the Coach before it is brought to the AD for approval. Form is required to be completed and turned in to Athletics the day prior to the event. Also to be completed when 'Private Vehicle Transportation' form T-30 is used.

Athletic Clearance Instructions
Instructions to register athletes via HomeCampus.com. We encourage all parent/guardian's and their athletes to register at HomeCampus.com for online athletic clearance to minimize errors, better facilitate the Ticket to Play process, and avoid delays for the upcoming school year.
Athletic PE Exemption Form
For 10th-12th grade students only. Students enrolled in any PE class are not concurrently eligible for a PE exemption. It's the student's responsibility to fulfill the team requirements {as designated by the coach} and to turn in paperwork by the deadline. It's coach's responsibility to notify Athletics that the student fulfilled the season with the team.

- The fall deadline is September 3, 2024 - Cheer (Sideline), Cross Country, Field Hockey,
Flag Football (G), Football, Golf (G), Tennis (G), Volleyball (G), and Water Polo (B). - The winter deadline is December 2, 2024 - Basketball (Boys, Girls), Cheer (Stunt), Roller Hockey, Soccer (Boys, Girls), Water Polo (G), and Wrestling (Boys, Girls).
- The spring deadline is March 11, 2025 - Badminton, Baseball, Golf (B), Gymnastics, Lacrosse (Boys, Girls), Sand Volleyball (G), Softball, Swim & Dive, Tennis (B), Track & Field, and Volleyball (B).
Authorization for Student to Carry Medication
If student uses any medication (prescription or over the counter) during the school day and/or while playing sports, parent/student/prescribing health care provider must complete PUSD Form H-26B {MAF}. Students with anaphylaxis allergies PUSD Form H-58 {AAEP}. Students with Asthma who need to carry an inhaler will need the Asthma Symptom Action Plan. Bring (all) the medication(s) for verification along with the completed form to the Athletic Assistant or Athletic Trainers. This will need to be completed annually.

Complete the two page health history forms prior to obtaining your physical exam, page three is to be completed by the CA licensed health care provider at the time of the exam. Clearance is required annually/seasonally for student athletes to obtain their Ticket to Play. Physicals are good for ONE year from date of examination and cannot expire during the sports season. {Must be valid prior to the season and valid through the conclusion of the season.}

In compliance with California Education Code 32221, you must certify that insurance coverage exists for your son/daughter and that this coverage will remain in effect throughout the time that he/she participates in sports. One option for your consideration is Pacific Educators, or via (800) 722-3365.

The Receptionist will either confirm Parent/Guardian are in the PUSD Volunteer database, or Parent/Guardians must complete the Westview Volunteer Paperwork including Level I - Megan's Law clearance, Code of Conduct, and provide a valid TB test result prior to your Private Vehicle Transportation Form being approved.
Once notification of Level 1 - Megan's Law approval is received by the Westview Receptionist, Parent/Guardian will be invited to provide the Private Vehicle Transportation Form (T-30), declarations page from vehicle insurance provider showing required minimums, and valid California drivers' license to the Athletic Assistant.

Transfer Athletes
Students who have attended another high school need to complete the following forms, turn the forms into Athletics, and schedule to meet with the Athletic Director asap once enrolled at Westview. CIF Form 510 and Westview's Student Athlete Transfer form. This may be a lengthy process, so the sooner you can meet with the AD the better.

We strongly encourage using AthleteClearance. If you select "alternate athletic packet" option, clearance and/or tryout could be delayed. If you have registered at HomeCampus.com "alternate athletic packet" is not required.
